Twist Placement

Question: Can the twist be at the very end or very beginning? Would that make the rest of the book less appealing? and can you have little twists within the story to compensate?

Answer: Very often you have a twist, small or large, at the start of each of act of a four-act structure as well as one at the end (for a total of five). They are also called the drivers.

Sometimes it's just the large twists that get noticed, but the others are still there.
